Coleman Transportation - Non-Emergency Medical Transportation (NEMT) Services - Terms and Conditions

1. Service Overview: Coleman Transportation provides Non-Emergency Medical Transportation (NEMT) services in the state of Florida. Our services are designed to assist individuals with transportation to medical appointments, therapy sessions, and other healthcare-related visits.

2. Eligibility: To utilize Coleman Transportation's NEMT services, individuals must meet the eligibility criteria established by the state of Florida and any applicable Medicaid or insurance provider guidelines.

3. Booking and Scheduling: Clients must schedule NEMT rides in advance by contacting Coleman Transportation's booking department. Rides are subject to availability, and clients are encouraged to schedule appointments well in advance to secure their preferred time slots.

4. Cancellation and Rescheduling: Clients are required to provide sufficient notice for cancellations or rescheduling of NEMT rides. Failure to provide timely notice may result in cancellation fees or restrictions on future bookings.

5. Transportation Policies: Clients are expected to adhere to all transportation policies and guidelines established by Coleman Transportation. This includes being ready for pick-up at the scheduled time, following any safety instructions provided by the driver, and respecting the rights of other passengers.

6. Medicaid Coverage: Coleman Transportation accepts Medicaid coverage for eligible individuals. Clients are responsible for verifying their Medicaid eligibility and coverage for NEMT services.

7. Privacy and Confidentiality: Coleman Transportation respects the privacy and confidentiality of our clients' personal and medical information. All client information is handled in accordance with applicable privacy laws and regulations.

8. Liability and Insurance: Coleman Transportation maintains appropriate liability insurance coverage for the protection of our clients and employees. Clients are encouraged to carry their own insurance coverage as applicable.
